Please note that several participants in this survey have had difficulties submitting this form automatically. As far as we can ascertain, these problems are usually the result of the conflicts between the email software used by the browser itself and other email software loaded on your computer. If you have difficulties, please let us know. Also, because of these difficulties, we have provided a downloadable form in ASCII format.
